GENERALLY

The accounts listed on page 1 of the authorization are covered by their individual terms and conditions, unless modified by this Authorization. If a transfer is made from a Savings or Money Market Account, we reserve the right to require not less than seven (7) days written notice of withdrawal. Withdrawals from a Savings or Money Market Account are limited to six (6) per statement cycle, transfers for overdraft protection are included in this limit; a service fee will apply each statement cycle in which more than six (6) withdrawals have been made. If the available balance in the transfer account is not sufficient to cover the overdraft amount, the remaining amount less any pending service fees in the transfer account will be transferred. The remaining overdraft amount may result in overdraft charges and/or returned items. We may use our rights and remedies under applicable law and our rules and regulations governing these types of accounts. These may include returning your checks unpaid and closing your account(s) by mailing a proper notice to you with a check equal to the balance in the account. You agree, in consideration of this service rendered by us, to indemnify (repay us for any loss) and hold us harmless (release us from any responsibility) from any liability or loss occurring due to the dishonor of any check presented which results from any charge made or refused to be made by us under this authorization. You agree to abide by our rules and regulations governing your account(s) as stated on your account agreement and as amended from time to time. We may take any security measures that we believe are necessary (such as recording telephone transfer conversations) without notice to you. If you do not have any overdraft protection plan(s) attached to your Checking Account, as a general practice we will not consider authorization/payment of transactions presented against your account if it does not contain sufficient collected funds. As described herein, an item or transaction that is returned and later represented to us may result in you incurring additional fees, such as multiple nonsufficient funds (NSF) fees each time that item or transaction is presented. Our normal practice will be to deny authorization/payment of any items presented for payment against insufficient funds in your account. The Bank cannot guarantee it will never exercise its discretion to authorize or pay a transaction you have initiated for which you do not have sufficient collected funds. MidWest One will charge the current fee whether an item is paid or returned. OVERDRAFT FLEX With your eligibility into Overdraft Flex, you may overdraw your MidWestOne Bank Account up to the overdraft limit of $50.00 without incurring any charges. Overdraft Flex can be used for any type of transaction made with your MidWestOne checking account. We will not authorize ATM and everyday debit card transactions that will overdraw your account without first receiving your affirmative consent to do so. Qualifying Conditions Your MidWestOne checking account must meet the listed conditions below (the "Qualifying Conditions") to qualify for Overdraft Flex. 1. Be enrolled in OOPS and meet the good standing criteria noted on page 1 of the authorization. 2. Be enrolled in an eligible checking account product. 3. Receive a total of $500.00 in Qualifying Direct Deposits (as defined below) into the MidWestOne Bank checking account in the previous thirty -five (35) days. a. A "Qualifying Direct Deposit" is defined as a direct deposit by ACH transfer of payroll, pension, or government benefits from your employer or a government agency. The recipient's name associated with such deposits must match the name of the MidWestOne checking account holder. 4. Qualifying Direct Deposits do not include the following: a. Transfers from an account held in your name at another bank, brokerage firm, or other financial institution whether from the MidWestOne Bank Account holder or a third party; or b. Transfers from digital wallets or Peer to Peer (P2P) services, including, but not limited to, PayPal, Venmo, Square, and Apple Pay Cash; c. Transfers made to the MidWestOne Mastercard Debit Card via Mastercard Direct, from digital wallets or P2P services, including, but not limited to, PayPal, Venmo, Cash App, Zelle ®, Google Pay, Facebook, or other similar transfers. d. Cash deposits made via Third Party Money Transfer Services, including, but not limited to, the Green Dot® Network. e. Mobile Deposit of checks via the MidWestOne Mobile App. f. MidWestOne to MidWestOne Transfers received in the MidWestOne Bank Account. g. Cash or check deposits made via ATM or at one of our branch locations. Overdraft Flex Terms and Conditions: Overdraft Flex will be removed from your account if any of the following situations occur: 1. If your account has not received qualified direct deposit(s) in the previous thirty-five (35) days. 2. If your account is overdrawn for any dollar amount for thirty-five (35) days or more 3. If OOPS is removed from your account due to misuse of the service. 4. If you voluntarily un-enroll from OOPS. 5. If your account is no longer considered in good standing as outlined on page 1 of the authorization. We reserve the right to revoke Overdraft Flex at any time for any reason.
