Confidentiality We will disclose information to third parties about your account or the transfers you make: (1) where it is necessary for completing transfers; or (2) in order to verify the existence and condition of your account for a third party, such as a credit bureau or merchant; or (3) in order to comply with government agency or court orders; or (4) as explained in the Privacy Disclosure contained elsewhere in our communications. Error Resolution Notice In case of errors or questions about your electronic transfers, call or write us using the contact information below, as soon as you can, if you think your statement or receipt is wrong or if you need more information about a transfer listed on the statement or receipt.

To report a lost or stolen check card after business hours call 800-383-8000 (Shazam®) or 800.247.4418 (MidWest One Service Center). Regarding ATM Receipt Message: Please note that if you have an overdraft line of credit or sweep account, this amount will not be included in your available balance; however, these funds will still be available to make purchases or ATM withdrawals. If you access your line of credit when making a purchase or ATM withdrawal, you will be subject to the terms and conditions (including fees and interest) noted in your agreement. Multiple Account Access Youmay attach ATM or debit cards up to three different checking accounts and up to three different savings accounts. Your “primary” checking account will be used for all debit card purchases. By attachingmultiple accounts to a card, you will be able to transfer funds between accounts with us at an ATM. It is your responsibility to define your accounts with us.
