Timeline and Checklist

Transition from Bank of Denver to MidWest One Bank

Please use this timeline and checklist, as well as other pages in this guide, to help you plan for your banking needs before and after March 11, 2024, when your accounts and services transition to MidWest One .

Thursday, March 7 or earlier

☐ Print your bill pay transaction history. This information will not be available after conversion.

Before Friday, March 8 Please ensure your email address is correct with us so you can receive detailed communications related to your online and mobile banking services.

Online, mobile, and telephone banking will not be available during the weekend.

☐ Print your online transaction history if you desire. ☐ Note your online banking username to make logging in easier on Monday. ☐ Print a copy of your bill pay transactions and billers. ☐ Record any existing recurring transfers through online or mobile banking and any alerts you have set up. Existing recurring transfer and alerts will need to be set up again after your first log in. ☐ Continue using your Bank of Denver ATM or debit card. At the end of the day on March 8, 2024, a special conversion statement will be mailed for all checking and savings accounts. Interest will be paid, and account service charges will be waived.

Our offices will be closed to prepare for the conversion. Online, mobile, and telephone banking will not be available.

Monday, March 11

Our offices will open for regular hours.

Bank of Denver ATMs will be converted to MidWest One machines and may be unavailable for brief periods.

Direct Deposit, ACH, and Automatic Payments

We are bringing two strong community banks together. This enables us to offer you the best of both banks. Any changes in account terms are specified in this guide. Account andtermschangeswill beeffectiveMonday, March 11, 2024. MidWest One offers a variety of banking options and benefits. If you wish to change the type of account you presently have to a different MidWest One account, simplycall or stop in afterMarch11 .We’realwayshappy to visit with youabout thedifferent types of accountswe have available and to help you choose the best one for you. If you have questions, you may continue to call your local bank office. See a list of locations and phone numbers on page 19 of this guide. You may also visit https://www.thebankofdenver.com for answers to frequently asked questions. Thank you for trusting us to continue to serve your banking needs! You may continue to use your existing supply of Bank of Denver checks until they are gone. When it is time to reorder, please contact your local bank office or call our Service Center for assistance. If you order checks through another vendor, you must provide the new routing number 073901233 when you place your next order after March 11, 2024. Read pages 10 to 16 to view how Bank of Denver checking accounts will transition to new corresponding MidWest One Bank checking accounts. Checks and Checking

If you have direct deposit, ACH, or automatic payments set up on any of your accounts, they will continue to occur as previously established. If you set up new direct deposit, ACH, or automatic payments after March 11, 2024, use the new routing number 073901233.

The MidWest One routing number can also be found on MidWest One checks, as shown here. Some companies may require you to complete a new authorization form and provide them with a voided check when setting up new entries.

Account Numbers

In most cases checking account numbers will not change. A small number of accounts were identified as having a duplicate account number. To avoid compromising data privacy, we issued new account numbers as necessary. Customers affected by duplicate account numbers were previously notified. If you did not receive a prior notification, your account numbers were not impacted.

Electronic Monitoring and Alerts

ATM and Debit Cards

Our online and mobile banking services give you the opportunity to customize account alerts. After March 11, you will need to re-establish any alerts you had at Bank of Denver through MidWest One Bank online or mobile banking. You can be notified via email, text message, or both if your account balance ever falls below a particular threshold or for other important events on your account. This service is free however text and data rates may apply.

You may continue to use your current Bank of Denver Visa® ATM or debit card for the foreseeable future. Your card will be replaced with a MidWest One branded Mastercard® debit card at a future date. The standard debit card daily limits will change on March 11, 2024, to $750 ATM cash withdrawals and $5,000 for card purchases. The standard ATM card daily limits will change on March 11, 2024, to $310 for cash withdrawals. MidWest One ’s online and mobile banking enables you to pay your bills, deposit checks, check your balance at a glance, receive alerts, manage your accounts, and more. On Monday, March 11, you may log in on the MidWest One online banking system or through the MidWest One Bank app. We will transfer your username to the MidWest One system. When you first log in, enter your existing username from Bank of Denver. We will provide you with your temporary password closer to March 11. You’ll be prompted to set up a new password and accept the terms and conditions. Mobile Deposit: You will have access to mobile deposit which will enable you to deposit checks up to $5,000 daily. If you do not currently use online banking, please contact us for assistance with enrollment and setting up your account. Your Bank of Denver payees, payments, and transfer information will carry over to MidWest One Bank. Your bill payment history will not carry over to MidWest One Bank. If you would like your recent bill payment history, please contact us at 303.572.3600 before March 7. Please check your payee and payment information for accuracy when you log in for the first time on or after March 11. Online and Mobile Banking Bill Pay and Recurring Payments and Transfers

Electronic Statements

MidWest One offers online secure electronic state- ments at no charge to you. By enrolling in electronic statements you will be able to view your statements faster, cut down on clutter, safeguard your private information, and “go green” by reducing waste.

Telephone Banking

You may continue to use the current Bank of Denver telephone banking service until March 8. When you call on or after March 11, call the MidWest One Telebanc service at 1.800.832.9322. The first time you call on or after March 11, you will need to use the last four digits of your Social Security Number to access Telebanc. You will then be prompted to change the PIN to something of your own choice.

FDIC Insurance

FDIC Insurance

The standard deposit insurance amount is $250,000 per depositor, per insured bank, for each account ownership category. Deposit accounts held at Bank of Denver prior to the merger with MidWest One will be separately insured by the FDIC until August 1, 2024 to give you an opportunity to restructure your accounts, should you choose to do so. This grace period is described in more detail below: • Checking, savings and money market accounts held at Bank of Denver will be insured separately by the FDIC up to applicable limits until August 1, 2024. • Bank of Denver CDs dated prior to August 1, 2024, will be separately insured by the FDIC up to applicable limits until the first maturity date after August 1, 2024. CDs that mature during the six-month interim period and are renewed for the same term and in the same dollar amount, (either with or without accrued interest), continue to be separately insured until the first maturity date after August 1, 2024. • CDs that are renewed on any other basis will be separately insured only until August 1, 2024. • Deposit accounts opened after August 1, 2024, may not be separately insured. • After August 1, 2024, accounts will continue to be insured up to applicable limits by the FDIC through MidWest One Bank.

Overdraft Services

Overdraft Protection Plans

be enrolled in OOPS with a $1,500 limit. You may choose to decline this service at any time. New accounts meeting product and ownership eligibility requirements opened on or after March 11, 2024, will automatically be enrolled into OOPS after the account has been open for 30 days. Cost: There is no cost to be enrolled in OOPS. We may charge an overdraft fee for any item we pay into overdraft. For additional details please see Occasional Overdraft Protection Service Guidelines located in the enclosed disclosure packet. If enrolled in OOPS, Consumer Accounts may be eligible for Overdraft Flex, we will not charge a fee if your account is overdrawn by $50 or less and you’ve met the qualifying conditions: Meet the “good standing” criteria for OOPS. An account in good standing exhibits, but not limited to, the following characteristics: Account has been opened for thirty (30) days, Account has a positive balance at least once every thirty (30) days, Account is not subject to any legal or administrative order. Be enrolled in an eligible consumer checking account product. Receive a total of $500 in qualified direct deposits into the MidWest One Bank checking account in the previous 35 days. A “qualified direct deposit” is defined as a direct deposit by ACH transfer of payroll, pension, or government benefits from your employer or government agency. The recipient’s name associated with such deposits must match the name of the MidWest One Bank checking account holder. Our fees will be added to any outstanding overdrafts you may have and will be applied against your OOPS limit. This service is offered at the discretion of MidWest One Bank and is a purely discretionary courtesy that we may provide for you. Your account will be charged the current fee for each item whether paid or returned unpaid.

You may choose one – or all – of the services below. If you select more than one option, the service will be activated in the numerical order you elect: 1. Existing automatic transfer from checking, savings, or money market accounts Transfers from checking or savings or money market accounts will be made in increments of $100 to cover each overdraft in your checking account. Debit card and ATM transactions will be authorized based on your available balance in your transfer account. 2. Existing automatic transfer from an eligible line of credit (subject to credit approval) Advances in increments of $100 on an eligible line of credit can be made to cover each overdraft on your checking account up to your credit limit. Debit card and ATM transactions will be authorized based on your available balance in your transfer account. Interest will be charged per your loan agreement on the outstanding balance on your line of credit. 3. Effective March 11, 2024 OOPS–Occasional Over draftProtection Service (subject to qualifications) MidWest One Bank offers an Occasional Overdraft Privilege Service on specific account types. If your account qualifies for OOPS, we will consider, without obligation on our part, paying items for which your account has insufficient or unavailable funds, instead of automatically returning those items unpaid. On March 11, 2024, your account will be enrolled in OOPS if it meets the eligibility requirements for product type and ownership type listed on the next page. Certificates of Deposit and Convenience Services

Certificates of Deposit (CD) and Individual Retirement Account (IRA) Certificates of Deposit MidWest One will honor the rates and terms on your CDs until they mature. At that time you will be able to renew at MidWest One ’s current rate. Once the CD matures, it may be subject to a new term and rate. You will receive a notice 10 days before your certificate renews which will state the duration of the automatic renewal. You will have 10 calendar days after the maturity date to change the renewal term. Change to Early Withdrawal Penalty, Effective March 11, 2024 As of March 11, 2024, the amount of principal withdrawn before maturity will be subject to early withdrawal penalty using the stated interest rate on all new and renewed certificates as follows:

Mobile Banking with Mobile Deposit —Deposit checks, transfer funds, pay bills, view account balances or transaction history all from your mobile device. Text, browser, or app —we offer three convenient ways to choose how to use Mobile Banking. Message and data rates may apply. Person-to-Person Payments —Zelle® lets you send money to people you know. It’s a fast, safe, and easy way to send money in minutes. Bank Your Change™ —With every purchase on your debit card, the change is rounded up to the next dollar, and is placed in your savings account automatically. Please refer to the Safe Deposit Box Fees on page 18 for information about any changes to safe deposit box fees that will be effective March 11, 2024.

Loans

MidWest One will acquire all of Bank of Denver’s consumer loans and will service your loan accounts as usual. You should continue to make payments in accordance with the terms outlined on your original loan. Effective March 11, 2024, payments made to your loans will be applied in the following order: principal, interest, escrow, late charges, and collection expense. After conversion, your loan will display on your credit report twice. There will be an entry with the Bank of Denver name, which will indicate it was transferred or

Your Ability to Withdraw Funds at MidWest One Bank

For determining the availability of your deposits, every day is a business day, except Saturdays, Sundays, and federal holidays. If you make a deposit before 6:00 p.m. CT or branch closing on a business day that we are open, we will consider that day to be the day of your deposit. However, if you make a deposit after 6:00 p.m. CT or branch closing or on a day we are not open, we will consider that the deposit was made on the next business day we are open. Deposits at Automated Teller Machines (ATMs) If you make a deposit at an ATM (as applicable) before 6:00 p.m. CT on a business day we are open, we will con- sider that day to be the day of your deposit. However, if you make a deposit at an ATM after 6:00 p.m. CT, or on a day we are not open, we will consider that the deposit was made on the next business day we are open. Funds from any deposit made at an ATM we do not own or operate will be made available on the second (2) business day after the day of your deposit. Some ATMs owned and operated by MidWest One Bank and located on bank premises do not accept deposits. Longer Delays May Apply Case-by-case delays. In some cases we will not make all of the funds that you deposit by check available to you on the first (1) business day after the day of your deposit. Depending on the type of check that you deposit, funds may not be available until the second (2) business day after the day of your deposit. The first $225 of your deposits, however, will be available on the first (1) business day. Special Rules for New Accounts If you are a new customer, the following special rules will apply during the first 30 days your account is open. Funds from electronic direct deposits to your account will be available on the day we receive the deposit. Funds from deposits of cash, wire transfers, and the first $5,525 of a day’s total deposits of cashier’s, certified, teller’s, travelers, and federal, state, and local government checks will be available on the first business day after the day of your deposit if the deposit meets certain conditions. For example, the checks must be payable to you. The excess over $5,525 will be available on the ninth (9) business day after the day of your deposit. If your deposit of these checks (other than a U.S. Treasury check), is not made in person to one of our employees, the first $5,525 will not be available until the second (2) business day after the day of your deposit. Funds from all other check deposits will be available on the eleventh (11) business day after the day of your deposit. For the first $225 of your deposits, next day availability does not apply to new accounts.
